Abstract : Background of Study: The role of chromium (Cr) in improving glucose tolerance has been reported since the sixties . Supplementation studies confirmed this in different countries .Since Chromium is a nutrient , only deficient subjects will benefit . Diabetes is a serious problem is Saudi Arabia , and the extent of chromium deficiency is not known. Objectives: To investigate chromium status in Saudi subjects with type I and type II diabetes and assess the effect of chromium supplementation. Methods: Seventy eight NIDDM and 30 IDDM patients were divided randomly into two groups each , and given Cr supplement as brewer's yeast and Crl3 sequentially with placebo in between, in a double-blind , cross over design of four stages, each lasting 8 weeks. At the beginning and end of each stage , subjects were weighed, their dietary data and drug dosage recorded, and blood samples collected for analysis of glucose (fasting and 2L post 75g glucose load) fructosamine and lipid profile. Results: Both supplements caused a significant decrease in the means of glucose (fasting and post glucose load) fructosamine and triglycerides, and increased the mean HDL-cholesterol in type II diabetic subjects. The mean drug dosage decreased slightly and significantly in the case of glibenclamide. However, in type I diabetic subjects, this was only noted when brewer's yeast was given. Conclusions: Chromium deficiency exists in Saudi subjects since chromium supplementation helped in the control of glucose and improved lipid variables. Chromium supplements are cheap and could help to cut cost of this chronic disorder. A larger scale study is needed to help decide on the appropriate chemical form and dosage required to achieve optimal response.
